File a complaint or ask a general question on the Allegheny County Self-Service Portal. Select Request Service/File Complaint, then select Health Department, and then select the appropriate program.

Speaker Request

If you are an organization, school, non-profit, etc., and you would like to have a representative from the Health Department speak or provide a lecture at your event, please complete the ACHD Lecture and Speaking Engagement Form.

Event Attendance Request

If you are simply interested in having a Health Department representative attend your event, please complete the ACHD Event Form.

Hosting a Vaccination Event

If you are interested in hosting a vaccination event in coordination with the Allegheny County Health Department, please complete theACHD Vaccine Event Form.
